Our mission as a for-profit and Licensed Home Health Agency is to provide paraprofessional services to clients in their day-to-day self-care activities. We are committed to providing high quality care by staff that recognize the need for each clients personalized support.
The Agency, under direction of the patient's physician and assessment of caseworkers provides the care needed to adequately maintain the patient's home. In the environment of his/her home, the patient will be cared for thoroughly by a staff dedicated to maximizing the "wellness" state of the patient.
The objectives of services provided by the Agency are to meet the physical and psycho-social needs of the patient in the time of short or long-term illness, or emotional crisis, which may threaten the normal pattern of life. Services provided will contribute to the prevention of the disintegration of individual or family life.
